Acknowledgment
The signer acknowledges that they have signed the document for its intended purpose.
Copy Certification
The notary certifies the copy made is a true and accurate copy.
RI vital records such as birth, death, and/or marriage certificates cannot be copied by notaries public. Copies must be ordered through the dept. of health.
Signature Witnessing
The signer signs the document within the presence of the notary.
Jurat
The signer swears under oath that everything in the document is true and accurate.
Oath / Affirmation
The oath taker verbally promises to tell the truth.
Please do not sign documents until in the presence of a notary public.
